Physical Therapist (Per-Diem) – Home Health King County

This position is responsible for therapy evaluations, treatment planning, implementation and discharge planning for clients in the home setting in accordance with established facility/agency policies while maintaining the highest degree of quality client care

Required qualifications:

  • Bachelor’s Degree in Physical Therapy from an institution approved by the State board.
  • Upon hire: Washington Physical Therapist License
  • Upon hire: National Provider BLS – American Heart Association
  • Upon hire: A valid driver’s license and proof of insurance per company policy will be required, or an alternate ability to travel to sites.
  • 2 years of therapy experience with adult physical disabilities.

Preferred qualifications:

  • Upon hire: Home Health King County: OASIS-C Certification; or Geriatric Clinical Specialist (GCS) by the American Board of Physical Therapy; or Neurological Clinical Specialist (NCS) by the American Physical Therapy Association (APTA); or Orthopedic Clinical Specialist (OCS) by the APTA.
  • 1 year of Therapy experience in home care or in related rehabilitation setting.
  • Experience with the geriatric population.

About the Team

Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.

HCC provides a full range of services and support for people of all ages, especially those facing chronic or life-limiting illnesses. Our core competencies include restorative care, longitudinal care and care through the end of life.
